For an economy SUV, yes — $65 one way including the driver and normal luggage. Larger vehicles cost more (see the table above). Tips are appreciated but never required or assumed.

Should I fly out of SFO or SJC from Palo Alto?

SJC is about 5 miles closer to Palo Alto and usually a calmer airport; SFO has far more direct routes, especially international. We drive both corridors at flat rates, so pick the flight, not the drive.

Can I book a pickup from an office instead of a hotel?
